IMPORTANT NOTE TO THE NOMINEE:

As you consider accepting your nomination for the AXA, please bear in mind that the award not only recognizes your achievements but (more importantly) also serves to inspire other alumnae and motivate them to strive for their very best.  Unlike other awards, AXA is not a competition among alumnae. We request a description of your achievements to allow us to determine that they meet the AXA criteria.


PURPOSE:

To recognize 10 CHS alumnae in North America whose life exemplifies “Veritas in Caritate” (Truth in Love) and therefore worthy of emulation, as we celebrate CHSNAF’s 28th Anniversary.


QUALIFICATIONS:

  • a. Must be a bona fide High School or College graduate of a CHS school in the Philippines. 
  • b. Must be a current resident or citizen of North America. 
  • c. Must be nominated by an alumna (not necessarily from the same CHSNAF Chapter).
  • d. May be a deceased alumna.
  • e. Must have reached out beyond her immediate family to others to foster the spirit of Caritas and made a difference in the lives of those whom she touched through her personal undertaking.
  • f. Must have excelled in her chosen field, career, ministry, apostolate, projects and/or activities.
  • g. Must not be one of the 100 CHSAF Awardees during the CHS Centennial Celebration nor an AXA awardee in any prior year.
  • h. Must not be a Member of the AXA Selection Committee.

  

REQUIREMENTS:

The attached nomination form should be received no later than July 31, 2019, by Awards Coordinator Lina Suarez-Umali, by email linaumali@hotmail.com, fax 713/932-0925 or mail to 6 Hilshire Grove, Houston, TX 77055. Up to 10 awardees will be recognized; the new awardees will be the top 10 nominees with the highest score.


JUDGING:

By August 6, 2019, the Coordinator will distribute a copy of the submitted nomination forms (without the Nominee’s name and Chapter) to the AXA Committee members made up of Arlene Torres, Pilar Bernardo, Babit Garcia and Darlene Troy Milo.  The Committee members will complete the Score sheets and conduct a conference call to finalize the selection by August 27, 2019.  By August 31, 2019, the awardees will be notified via email.


   

SCORING CRITERIA:

1. Scope/Coverage of those benefitting from candidate’s career/project/activity(ies): max. total = 20 points

  • Limited coverage (family, parish): up to 8 points, if work-related; up to 1points, if not work-related.
  • Expanded coverage (city, county): up to 12 points, if work-related; up to 15 points, if not work-related.
  • Wide coverage (regional, national, global): up to 16 points, if work-related; up to 20 points, if not work-related.


2.  Time Span of career/project/activity(ies): maximum total = 20 points.

  • Up to two years: up to 8 points, if work-related; up to 10 points, if not work-related.
  • More than two years but less than five years: up to 12 points, if work-related; up to 15 points, if not work-related.
  • More than five years: up to 16 points, if work-related; up to 20 points, if not  work-related.


3. Qualities Demonstrated in career/project/activity(ies): maximum total = 20 points

  • Leadership, team spirit, creativity, compassion, integrity, selflessness/generosity, etc.: up to 16 points, if work-related; up to 20 points, if not work-related.


4. Complexity/Degree of Difficulty or Uniqueness of career/project/activity(ies): maximum total = 20 points

  • Degree  of difficulty (education/skills/training requirements; need for overcoming bureaucratic hurdles or apathy/disinterest, family/group conflicts); number of people involved (performed alone or required group effort): up to 16 points, if work-related; up to 20 points, if not work-related.


5. Special Circumstances To Be Considered: maximum total = 20 points

  • Other  factors that could be considered: up to 16 points, if work-related; up to 20 points, if not work-related. 


6. Bonus Point: maximum total = 5 points 

  • Attendance/participation in CHSNAF-sponsored events/activities.
